Why Medi-QR matters

In an emergency, seconds count. Medi-QR puts critical health information where it’s easy to find—on a wallet card and wearable badge— so the right people can act quickly and confidently.

Faster emergency decisions

Provide key details like allergies, medications, conditions, and emergency contacts—so responders can avoid dangerous delays.

Works without an account

No logins to remember and no app required to create your tag. Just fill out the form and get your printable PDF.

Offline-ready access

Your QR code contains the information you chose to include—so it’s available even when there’s no internet connection.

Easy to carry and display

Keep a wallet card with you and place the badge where it’s easy to find—purse, backpack, luggage, wheelchair, or lanyard.

Great for caregivers and families

Helpful for seniors, children, and anyone with ongoing medical needs. Peace of mind for loved ones who may not be present.

Simple, scalable, and affordable

Ideal for individuals and also for groups like schools, care facilities, missions, and community organizations.

Best places to keep your Medi-QR

  • Wallet card: Keep in your wallet for daily carry
  • Badge: Clip to a lanyard, purse, backpack, or wheelchair
  • Home copy: Place on your refrigerator or near your front door
  • Vehicle copy: Keep in your glove box or visor

Quick questions

Do I need an app to create a Medi-QR badge?

No. You can create and print your Medi-QR from the website. In an emergency, responders scan the code to view the information encoded in the QR code with a QR Reader app on their smart phone.

Is my data stored on your website?

Medi-QR is designed to be privacy-conscious. The QR code contains only the information you choose to include. We do not store any medical data!

What should I print it on?

Standard paper works, but cardstock is best. Many users laminate the wallet card and badge for durability.
